Job Purpose:
To support delivery of the Healthy Child Programme (HCP) 0-19 (DH 2010) thereby
improving outcomes for children and families.
To contribute to the Public Health Agenda, meet Public Health targets and reduce health
inequalities in partnership with key agencies meeting National agendas.
To promote the protection of children, adhering to the Local Safeguarding Childrens Board
policies and procedures.
To work under the supervision of band 6 School NurseHealth Visitor within a locality School
Nursing Health Visiting team, and with partner agencies where appropriate, e.g. Early Help
networks, Children's Centres, school leadership teams.
To work as part of a team to ensure that Key Performance Indicator targets that are
set/negotiated by Local Area Teams/Commissioners for the service are met and evidenced.
To support service development, standard setting, clinical audit and bench marking, deliver a
high quality needs led, safe and effective service to all client groups.
This role requires the ability to fulfil all spoken aspects of the role with confidence and
fluency in English.
Main duties and responsibilities:
Participate in the assessment of health needs and formulation of care plans for children,
young people, their families and communities. This includes the implementation of
programmes of care, offering support and advice to parents and families in the home, clinic,
school or community setting ie: parenting skills, public health, group work and community
clinics.
In collaboration with the public health nursing team, identify individuals and groups within the
community where collaborative working and advice would forward the interests of public
health and promote healthy living, in particular seeking to identify disadvantaged groups and
address health inequity.
Offers advice and support to children, young people and adults in accordance with evidence
based practice.
To actively take responsibility for appropriately identifying, accessing and participating in
supervision, according to individual need i.e.: Child Protection, Restorative, and Clinical.
Support the delivery of key performance indicators by working collectively across wider
geographical areas.
Under the direction of the Specialist Community Public Health Nurse Specialist completing
the Early Help Assessment (EHA), working with the multi-agency team in supporting children
in need and those at risk of significant harm, including participating in case conferences and
other meetings and writing reports where appropriate. This includes undertaking the role of
lead professional or key worker for children and their families with the most complex needs.
Is aware of and complies with Devon County Councils policies, protocols, guidelines and
procedures, including policies on Health and Safety, Fire Regulations and Child Protection
policies and procedures, with support and guidance from manager and Named Nurse
Safeguarding Children.
Attends and contributes to Public Health Nursing team meetings to share knowledge and
identify areas of potential development for the service relating to Clinical Governance.
Where appropriate, manages and maintains equipment, eg: ensuring servicing of scales at
the required intervals. Has due regard to efficient and effective managements of resources,
eg: accurate prediction and maintenance of supplies, informing line manager of any
difficulties encountered.
Monitors own performance against agreed objectives through the annual process of review
and personal development plans and Nursing and Midwifery Council regulations, maintaining
own portfolio.
Participates in the development and teaching of staff and students, and in the induction of
new staff, eg: work based experience and training of staff at Band 4.
Takes responsibility for participating in regular, planned supervision and reflective practice.
Maintains and develops own professional expertise by arranging and attending meetings,
study days and in service training, including mandatory training as agreed with the manager
and professional lead.
Supported by the Specialist Community Public Health Nurse, Child Protection Supervisor
and Named Nurse for Safeguarding Children, contributes to reports on own involvement with
families for relevant agencies and multi-agency meetings, including Child Protection Case
Conferences, court reports and multi-disciplinary conferences on children with special needs
according to protocols established by Childrens services.
Keeps a daily electronic diary containing accurate record of work content and travel.
Initiate and participate in data collection, research, audit, surveys and service user feedback.
Develop and maintain effective communication networks and working relationships, with
service users, health care professionals and other statutory and voluntary organisations. In
collaboration with the Specialist Community Public Health Nurse, shares information with
other agencies, voluntary bodies and multi-disciplinary teams.
Patient Care Responsibilities
Maintain clear and accurate records as per standards and guidelines, ensuring clear
analysis and outcomes for the child.
To undertake clinical activities within the scope of professional practice
Plan, implement and evaluate health enhancing activities to meet health needs of the local
population within scope of own role and commissioned service through home visiting,
community development, clinics, workshops, support groups and parenting programmes,
focusing on key national and local targets.
To develop a workload profile, identifying needs as Universal, Universal Plus, Universal
Partnership Plus.
To identify children and families in need of early help through the Early Help Assessment
and Right 4 Children system.To act as Lead Professional as required and engage in SEND
pathway.
Act in accordance with current local and Trust guidelines/policies to protect children at risk.
Takes professional responsibility and accountability in the joint decision making process
within the Child Protection arena.
Communicates effectively with people who may
themselves have communication difficulties, including willingness to seek out and use
resources to help those whose first language is not English or those with sensory or learning
difficulties.
Communicates in such a way as to empower, motivate and/or support individuals, families
and other members of staff, judging which approach is most likely to be effective in
addressing the current health need or situation.
Is aware of and respects responsibilities involved in the confidential nature of the work and
an awareness of complex issues surrounding confidentiality and consent for children and
young people eg: contraception, teenage pregnancy, parental responsibility.
