Community Staff Nurse- Night Team

Band 5: £28,407 - £34,581

Various Hours - 2/3/4 Nights per week

Contract - Permanent

We are currently recruiting for registered nurses to work in the Community Nursing night team,which cover areas across North Kirklees and Greater Huddersfield.

We offer a fantastic preceptorship programm, and you will be fully supported to complete all the clinical competencies within our positive and hardworking teams.

Main duties of the job

Working within the Community Nursing Night team, you will undertake duties involving delivery of nursing services and holistic assessments of patients health needs using a person centered approach.

This includes providing quality and seamless nursing care to housebound patients and being proactive in coordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long-term care institutions.

In addition, you will have the opportunity to be involved in facilitating self-care for patients with long term conditions, health promotion and innovative activities and projects that enhance patient service delivery.

An enthusiasm to support our teams to deliver high quality services is a must for this role, and you will also need to demonstrate excellent communication and interpersonal skills with the ability to be adaptable, reliable, and approachable.

The service is able to offer some flexibility with working days in regard to career responsibilities and work life balance , but there will be an element of some weekend working, shifts times are 21.00 08.00.
