We welcome applications from Registered Mental Health Nurses, Social Workers, Allied Health Professionals (AHP) including Occupational Therapists or Art/ Music Therapists

You will have a broad depth of mental health experience working with individuals who have severe and enduring mental illness.

You will be responsible for care-coordinating a small caseload of service users with complex long-term mental health problems who find it difficult to engage with services. You will have experience in assessing and formulating individual intervention plans and outcome measures for adults with complex mental health problems and have current knowledge of NHS policies and practices in mental health. You will work in a strengths-based and recovery-focused way to support service users to meet their mental health needs.

Main duties of the job

You will possess excellent communication skills, essential for fostering relationships and engaging with individuals from diverse cultural backgrounds and various partner agencies. You will actively participate in weekly multidisciplinary team meetings and collaborate with colleagues to assess care plans and harm-reduction strategies, embracing a team-wide approach. As a positive role model, you will demonstrate a dedication to cultivating a learning culture from incidents and appreciating reciprocal feedback. Additionally, you will oversee supervision arrangements, guaranteeing that practitioners have opportunities to enhance their skills, expand their knowledge, and remain abreast of contemporary practices.

The transformation will happen in staggered stages; Hampshire CAMHS, part of Sussex Partnership NHS Foundation Trust joined Southern Health in February 2024, with Isle of Wight NHS Trust mental health and community services transitioning in early May 2024. The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.

Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
