Job summary

Enfield Unity Primary Care Network is the second largest PCN in England representing over 158,000 patients across 17 GP Practice sites. Our aim is to improve services and outcomes for patients by supplying a well-trained, motivated workforce.

We are seeking a qualified Health and Wellbeing Coach with knowledge of social prescribing to join our team. The ideal candidate will hold qualifications in nutrition and weight management and have experience working with individuals dealing with eating disorders and those looking to improve their lifestyle and eating habits through the power of social connections and community resources.

Main duties of the job

Support adults with health issues, especially those on a limited budget, in meeting their nutritional needs.

Provide motivational coaching and accurate nutrition education to promote healthy eating.

Incorporate appropriate and accessible exercise recommendations for patients, both at home and externally.

Conduct both one-on-one sessions and group sessions, both in person and remotely.

Utilize social prescribing techniques to connect patients with community resources, support groups, and activities that enhance their overall wellbeing.

Take referrals from PCN practices, assessing individual patients and managing a patient caseload, providing signposting to different services and connections with community groups and services. These services may include befriending, weight management, mental health support, smoking cessation, local social groups, leisure centres and volunteering, forging working relationships and creating referral pathways with VCSE organisations and many more.

About us

The successful applicants will be based over one or two of Enfield Unity PCN's 17 sites.

Medicus Health Partners (MHP) is the lead practice for Unity PCN and will be the employing organisation.

The successful applicant will have access to the training, support, and governance systems in place at MHP.

Applications are welcome from those with existing primary care or GP practice experience, or applicants that have a keen interest to develop clinical experience in primary care.

Job responsibilities

You will be confident to work 1:1 with patients in a GP setting; using behavioral techniques such as motivational interviewing to support them to identify, set, and meet achievable goals to improve their physical and emotional well-being.

We are looking for someone with relevant qualifications, skills and resilience to work with GPs and practice staff, helping people by addressing their needs in a non-medical way.

Your role is a key part of our growing and progressive PCN, working to provide an enhanced care experience, with a view to improving the service.

Person Specification

Qualifications

Essential

  • Higher level of education (e.g. NVQ level 3 or A levels at minimum or equivalent) in Health Coaching.
  • Completion of E-learning for health or other accredited program for Social Prescribers. Demonstrate commitment to personal and professional development. Essential experience in working with individuals dealing with eating disorders or poor lifestyle patterns affecting health.
  • Experience in social prescribing or a willingness to learn and implement social prescribing techniques.

Desirable

  • Experience with EMIS clinical systems.
  • Previous experience working in primary care, the NHS, or Social Care.
  • Experience working with people with learning disabilities and/or serious mental illness.
  • Enthusiastic, reliable, and compassionate.
  • Excellent interpersonal skills.
