Do you want to work in an integrated setting in primary care with colleagues from primary care, mental health services, including the voluntary and community sector, to improve access to support for people with mental health issues?

As part of our services across Oxfordshire, we are implementing Keystone Mental Health & Wellbeing Hubs to support GP practices with Mental Health assessment and support for patients in their communities.

We have created 4 new and exciting posts, all within each of the three City & NE Keystone Mental Health Teams (KMHTs). 2 posts to be based in the Keystone Oxford Hub (Cowley Road) and 2 posts to be based in the Keystone Kidlington Hub.

Main duties of the job

We are looking for 4 MH integrated hub clinicians who are experienced in delivering evidence-based interventions in a community setting, with a passion for partnership approaches to care for adults living with serious mental illness.

These roles need people who are innovative and want to help achieve our vision for cross-sector integrated pathways, removing existing barriers, to deliver better access to dedicated mental health support in local, non-stigmatised community settings.

Job description

Job responsibilities

  • Supporting the Clinical Lead in the development and implementation of the service and your role within it.
  • Supervision and leadership for appropriate staff identified
  • Screen, triage, assess and coordinate referrals
  • Provide brief support and time limited evidence-based interventions,
  • Support patients ready to transition to primary care and develop care plans and prepare people for step down.
  • Map community resources and assets that are available for those with SMI within the specific PCN geography.
  • To support collaborative working between the full range of physical health and mental health services

Person Specification

Qualifications

Essential

  • Current professional registration: Registered Mental Health Nurse, Occupational Therapist

Desirable

  • Post graduate education specifically relating to client group

Experience

Essential

  • Community experience in mental health services for adults and older age adults with severe / significant mental illness

Desirable

  • Experience of working in primary and secondary mental health services.

Knowledge

Essential

  • Specialist knowledge of person-centred care and its application.
  • Specialist knowledge of psychosocial interventions and their appropriate use
  • Understanding of the mental health needs of adults.

Desirable

  • Specialist knowledge relating to MH needs of young adults age 18-25 and to the needs of those diagnosed with, or with presenting traits of, personality disorder
