Dougie Mac are currently looking to recruit Registered Nurses to our developing Young Adult Service. Successful candidates will share our passion in providing excellent holistic and person-centred care to young adults and their families. Main duties of the job

To work as an integral part of the Young Adult Team: assessing, implementing and evaluating nursing care for service users living with palliative or complex health conditions. To complete a wide range of clinical interventions To act as a mentor / role model to Care Support Workers To support young adults in all aspects of living their lives, including support with activities of daily living, social and leisure opportunities. The right candidate will possess excellent communication and interpersonal skills, with the drive to ensure our young adults and their families receive the highest possible standards of care. Job responsibilities

Job Summary: To work as an integral part of the Young Adult Service: assessing, implementing and evaluating person centred care for service users living with palliative or complex health conditions. Work in partnership with other professionals and agencies to provide holistic, person centred care to service users and their families. To act in an advisory role to other professionals (both within and outside of the organisation) in respect of the impact of living with life limiting / life threatening and / or complex health conditions on young adults and their families. Duties & Responsibilities: Act as a role model within the Young Adult Team, motivating staff and setting an appropriate example. Mentor, train and assess competence of Care Support Workers. Shift coordinator To contribute to the training, education and support of a range of students on clinical placement with the service. Act as keyworker to a group of young adults. Responsibility for patient care Assess, plan, implement and evaluate plans of care for service users in partnership with them, their parents/carers (where appropriate), and other professionals. Provide effective care, treatment and management within the Young Adult Unit and/or community setting. Ensure all documentation is complete and kept up to date in accordance with policy. Liaise with healthcare and social care professionals across organisations to ensure an integrated package of care and support, designed to meet the specific needs of the service user, is delivered and co-coordinated. Ensure care delivered is delivered to predetermined standards. Work in partnership to actively involve service users and their parents/carers (where appropriate) in the implementation and evaluation of care plans. Provide information for service users and/or their families as their condition progresses, ensuring they are able to make informed decisions. Responsibility for Resources Promote the safe and effective use of equipment and resources Inform and liaise with Head of Service in regard to the needs of the service, both physical resources and skill mix. Have a good knowledge of all available resources and equipment. Strategic and service responsibilities Attend meetings relevant to post and on behalf of Head of Service or organisation Implement changes to established practices and/or policies Participate in service development under direction of the Senior Management Team to ensure support is safe, effective and efficient. Participate in and conduct audits of the service in partnership with other members of the Young Adult Service Team. Ensure practice is clinically effective and evidence based. Responsible for own continuing professional development. Maintain own revalidation and registration Work within the NMC Code and professional guidance

Person Specification

Qualifications

Essential

  • Registered Adult Nurse (RN), Registered Learning Disability Nurse (RNLD) or Registered Childrens Nurse (RNCB)

Desirable

  • Completion of PARE Supervisor Preparation and Guidance

Experience

Essential

  • Experience of working within a multi-disciplinary team.
  • Experience of working with service users who have additional communication needs, learning disabilities and / or physical health needs.
  • Transferable nursing knowledge and skills that can be adapted to caring for young adults living with life limiting conditions / complex health needs.
  • Experience of coordinating shifts / delegating tasks.

Desirable

  • Experience of multi-agency working with health colleagues, social care, and the voluntary sector.
  • Experience in caring for adults or children with complex health needs or palliative healthcare needs.
  • Experience of supervising / mentoring others.

  • Ability to drive is highly desirable due to the community aspect of this role
