The Planned Care Division is led by the Divisional Medical Director, Divisional Director of Operations and Divisional Director of Nursing using the triumvirate model of leadership. The Key Services delivered by this Division are Emergency and Elective Surgery, Intensive Care and Burns all of which are delivered through the busy but well organised and supported wards and departments.

We are looking for an enthusiastic and innovative Clinical Nurse Specialist for trauma and Orthopaedics, who will help support us through the challenges and opportunities that lie ahead.Due to maternity leave, this exciting opportunity has arisen for a talented individual with multiple skills and an appetite for delivering a first class nursing service to join the team in one of the UKs most successful and progressive NHS Foundation Trusts. The role entailspromoting excellence in nursing practice through the integration of expert clinical practice, education, consultancy, acting as a resource and commitment to nursing research.

Main duties of the job

The clinical nurse specialist for Trauma and Orthopaedics functions within a field of practice that focuses on the needs of the patient and encompasses interaction with others in delivering nursing, health and social care. The CNS also has a key role in the collection of data. The CNS is accountable for the total plan of care for groups of patients and this includes assessment, treatment and/or referral or discharge. The post-holder is expected to promote excellence in nursing practice and develop the role through the integration.

Working autonomously whilst working as part of a multi-disciplinary team, assessing and managing caseload of orthopaedic patients. Maintain effective communication with patients, carers and professionals to ensure seamless service delivery

To be responsible for coordinating the orthopaedic MDT meetings to ensure that all patient cases are discussed and managed within a multi-disciplinary setting.

As the specialist nurse member of the MDT to provide expert knowledge and ensure there is adequate psychological support, information and appropriate onward referral of patients to other health care professionals and external agencies, and to audit the service.

To be responsible for improving the pathway of care for orthopaedic patients.

To act as an advocate for patients and to raise the awareness of the needs of this specific patient group. To set up, coordinate and manage monthly support groups for patients, their carers and relatives.

Job description

Job responsibilities

To be responsible for improving access for screening, investigation, treatment and follow-up of patients.

To be responsible for managing a hip fracture follow up clinic with review at 120 days post surgery telephone or face to face clinic visit according to Royal College of Surgeon guidelines

To provide expert nursing and support for all members of the multi-professional team regarding the cart of individuals.

To use advanced examination and assessment skills to complete a comprehensive systematic assessment in a defined group of patients. This includes taking a patient history which explores physical, social, emotional, spiritual, psychological and environmental components

To order and interpret appropriate laboratory and radiological investigations and respond accordingly

To supply and administer treatment and medication in accordance with Trust guidelines

To be able to identify signs and symptoms of unstable or abnormal health conditions and responds appropriately.

To provide telephone advice to patients, their carers and primary care teams and arrange rapid referral and access to the orthopaedic service as required

To participate in discharge planning ensuring that the necessary arrangements and referrals are made to ensure continuity of care and patient/family support

To maintain a high profile in the clinical area, acting as a role model for expert clinical nursing practice and for other healthcare professionals

To recognise opportunities to develop own scope of practice initiatives

To assist in establishing, implementing and monitoring Trust guidelines, policies and protocols

To produce, implement and evaluate standards of nursing care and liaise with other orthopaedic CNSs across the Network/UK to implement agreed standards of care and clinical practice

To implement and disseminate change through policy and guideline development based on evidence.

Education/Leadership

To contribute to formal and informal education provision. To actively participate in the provision of education for patients, carers, relatives and staff groups (e.g. nurses and members of the MDT), and to evaluate education programmes based on evidence based practice in relation to orthopaedics.

To work collaboratively on education strategies which support services and orthopaedic service development.

To participate in teaching and assessing in clinical practice, including mentoring and preceptorship of nurses undertaking post registration courses in orthopaedics.

To discuss with patients any orthopaedic conditions and treatments on their lifestyle, offering health promotion advice, strategies for maintaining quality of life and activities of daily living as well as patient participation in their care.

Contributes to the setting up and running of Trust study days.

To establish links with other CNSs in order to share and/or learn from examples of best practice.

Research/audit/quality

To be proactive in ongoing audit in order to assess effectiveness of service

To participate as appropriate in relevant research studies

To regularly audit the role of the Orthopaedic Clinical Nurse Specialist, eliciting the views of patients, carers and professionals to ensure a high quality service

To critically evaluate research and audit findings and disseminate these findings to clinical practice and members of the MDT as appropriate in order to develop best practice and perform service improvement initiatives.

To develop baseline research skills which can be utilised in the advancement of the body of evidence supporting clinical decision making in orthopaedics.

To generate questions for research arising out of practice and to collaborate with relevant people in order to investigate these questions thereby acting as a research generator and coordinator.

To initiate and undertake primary nursing research and ensure that patients benefit from evidence based practice

To attend relevant departmental and Directorate meetings relating to clinical governance and to contribute to the clinical governance agenda.

To contribute to the development of orthopaedic nursing care locally and nationally by participating in specific groups and committees
