Job summary

We are looking for a senior AHP to lead the Senior Adult Oncology service who will be a proven expert in providing specialist interventions and providing pre and rehabilitation to complex older patients during anti cancer treatments as well as aiding cancer treatment decisions in complex older patients

The Senior Adult Oncology Rehabilitation Team Lead will be responsible for leading and coordinating the service at The Christie NHS Foundation Trust for this patient cohort. and will work within the MDT to provide outstanding evidence based clinical practice and co lead, with the Clinical Director, to develop the service strategy, quality standards and effective pathways of care

The post holder will have opportunity to contribute, facilitate, design and lead quality improvement projects with the aim of enhancing and promoting the value of the AHP role within cancer care.

Main duties of the job

To provide operational management and an effective and visible leadership by prioritizing and deputizing the team workload, developing effective working relationships across the team, ensuring clarity of roles, challenging poor practice and taking steps to ensure a productive team with high standards of patient care.

To work with the Clinical Director/ Geriatrician and service/ AHP leads in developing the strategic and operational management and aims of the service

To plan, manage and coordinate the care provided by the Senior Adult Oncology multidisciplinary team for older patients with cancer and frailty planned to start or undergoing anti-cancer treatments.

To act as a lead and specialist AHP resource within the field of senior adult oncology rehabilitation. This uses wide ranging expert clinical skills and knowledge necessary to the post holder's speciality and the application of these to the clinical setting

To propose pathway changes concerning the AHP management of patients and to guide the team to provide an efficient and effective service

DUTIES AND RESPONSIBILITIES

CLINICAL

Plan, manage and coordinate the care provided by the Senior Adult Oncology multidisciplinary team for older patients with cancer and frailty planned to start or undergoing anti-cancer treatments.

Provide operational management and an effective and visible leadership by prioritizing and deputizing the team workload, developing effective working relationships across the team, ensuring clarity of roles, challenging poor practice and taking steps to ensure a productive team with high standards of patient care.

To liaise between specialist and local rehabilitation teams to ensure that care for people with senior adult oncology needs is transferred efficiently across organisational boundaries.

To act as a lead and specialist AHP resource within the field of senior adult oncology rehabilitation. This uses wide ranging expert clinical skills and knowledge necessary to the post holders speciality and the application of these to the clinical setting.

  • To provide spontaneous and planned advice, teaching and instruction to relatives, carers and other professionals, to promote understanding of the aims of rehabilitation and to ensure a consistent approach to patient care.
  • To be professionally and legally accountable for all aspects of own work, including the management of patients
  • To assess, diagnose, develop, plan and implement highly specialist and individualised rehabilitation programmes where appropriate.
  • To provide appropriate highly specialist interventions and to monitor and evaluate clinical outcomes.
  • To use expert clinical reasoning skills and assessment techniques to provide and communicate an accurate diagnosis for patients with complex problems and place this within the context of their cancer and its treatment.
  • To use expert listening skills to understand the physical, cognitive, psychosocial, emotional and psychological needs of patients and carers and incorporate these into treatment planning. This includes the ability to interpret and respond to patients unsaid needs, to respond to an opening that the patient may provide and to refer patients to the appropriate health care professional to address this need.
  • To plan and provide individual treatment programme incorporating patient focused goals, outcome measures and onward referral This will be based on specialist knowledge of techniques and treatment options, a sound knowledge of evidence-based practice and a sound knowledge of cancer and its treatment.
  • To use a range of verbal and non-verbal communication skills and strategies to communicate effectively with patients, for example, patients may have a hearing loss, visual impairment, affected speech, cognitive problems or have a learning disability.
  • To evaluate patient progress and to re-assess and alter treatment programmes if required.
  • To advise on appropriate clinical outcome measures, to evaluate clinical outcomes of interventions and to assist other staff in carrying this out.
  • To use advanced communication skills to work with patients and carers in highly complex and/or sensitive situations.
  • Use advanced communication skills and expert clinical reasoning to enable the patient and carers to adapt psychologically to disease, role changes and disability and to set realistic rehabilitation goals.
  • To advise and support other healthcare staff to develop advanced communication and clinical reasoning skills to work with patients and carers in complex/sensitive situations, enabling them to adapt to any physical, psychological and emotional impact.
  • To attend the relevant multi-disciplinary team (MDT) meetings and case conferences to ensure the delivery of a co-ordinated multi-disciplinary rehabilitation Senior Adult Oncology service.
  • To use moderate physical effort, manual handling and coordination if necessary to assess and treat patients.
  • To use advanced personal coping strategies to work with a specialist caseload in the oncology setting.
  • To lead on the development of specialised care protocols/pathways relating to a range of issues to improve patient care.

13. Monitor the care provided by the multidisciplinary team and the impact of the service, identifying opportunities for improvement both within the Senior Adult Oncology team and the wider Trust.

Work within the Senior Adult Oncology multidisciplinary team, providing early and pro-active Comprehensive Geriatric Assessment (CGA) to selected cancer patients with frailty.

Contribute to the multi-professional team primarily through outpatient clinics and MDTs, but also by supporting selected inpatients.

Collaborate with the different supportive oncology teams and specialized oncology teams at The Christie.

Co-lead in shaping and optimizing the model of care provided by the Senior Adult Oncology Team.

Provide regular progress updates reports and work collaboratively with the other members of the service, in particular the Clinical Director, Geriatrician and the Implementation Lead in evaluating the benefits and effectiveness of the service implementation, as well as redesigning solutions and working across boundaries to achieve effective improvement outcomes.

Maintain high standards of data documentation (accurate, comprehensive and up to date) of all the required clinical and operational data, in line with national standards the trust policy, providing the necessary KPIs and outcomes data.

Attend and participate in team and departmental meetings when required.

PROFESSIONAL

To demonstrate a robust understanding of clinical governance, clinical risk, and to be able to apply this understanding to clinical practice.

Promote and participate in all aspects of clinical governance and risk management, including the development of SOPs, protocols, policies and standards to ensure they remain in line with evidence-based practice.

To be responsible for maintaining own capability and competency to practice through CPD activities and maintain a portfolio which reflects personal development in order to maintain HCPC and professional registration.

To ensure that own clinical practice and that of staff and students under professional supervision meet the required standards of professional practice.

To disseminate knowledge of specialist profession in an oncology setting by participating in the organisation of and presenting at courses, study days and conferences locally, regionally, nationally and internationally.

To undertake annual performance development reviews with staff.

To provide line management to qualified and non-qualified staff within the team.

To provide an AHP professional contribution into multidisciplinary service delivery and policy projects within the trust (e.g., implementation of NICE guidance documents)

To raise awareness of the role of the senior AHP in oncology.

To communicate potentially difficult clinical or non-clinical information to staff. This may include feedback about individual performance or dealing with conflicting advice and opinions about clinical situations.

To act as a role model and ensure compliance with The Christies values and behaviours.

Anticipate the consequences of national and government guidelines and policy on the Senior Adult Oncology service, and strategically recommend changes to practice and pathways to meet the changing and future needs of the service.

Identify own training and personal development needs, demonstrating a commitment to continuous personal and professional development (CPD) and maintain competency and to ensure best practice.

SERVICE DEVELOPMENT

To manage service development around AHP input into the Senior Adult Oncology service.

To utilise clinical expertise for ensuring that quality standards and effectiveness of patient care are continually improved within the clinical section.

To ensure that all staff within the clinical team implement policy and service developmental changes.

To propose pathway changes concerning the AHP management of patients and to guide the team to provide an efficient and effective service.

To work with the Clinical Director/ Geriatrician and service/ AHP leads in the developing the strategic and operational management of the services.

Co-lead on service evaluation and regular auditing process, promoting evidence-based strategies and clinical care of excellence.

Support the Clinical Director in the development of a service strategy and determining key deliverables for the Senior Adult Oncology Service

Actively seek feedback and promote involvement from patients, families, carers, communities and colleagues in the co-production of service improvements and taking this forward in developing tangible service improvements.

PLANNING AND ORGANISATION

To operationally manage and co lead a clinical caseload to meet competing service and patient priorities, frequently readjusting plans as situations change/arise and balancing other patient related and professional activities in accordance with professional and service standards.

To liaise across multi professional groups to ensure that the clinical service delivery is co-ordinated with others who may be involved in patient care.

To provide a safe, efficient and effective AHP service to patients within the clinical area by responsible and appropriate management
