Join our dynamic team at Worcester City Primary Care Network (PCN), where we are dedicated to improving patient outcomes through innovative pharmaceutical care. We are currently seeking a passionate PCN Clinical Pharmacist to join our team on a full-time basis.
Main duties of the job
As a PCN Clinical Pharmacist, you will provide expert clinical medicines review, address public health needs, and optimize patient medication regimens across our network. You will play a pivotal role in reducing poly-pharmacy, reconciling medicines post-hospital discharge, and ensuring adherence to best practice guidelines.

Job responsibilities
Core Responsibilities:
Conduct medication reviews, focusing on high-risk groups and chronic conditions.
Oversee dispensary operations and repeat prescribing reauthorizations.
Collaborate with community and hospital pharmacy colleagues to optimize patient care.
Implement medicines optimization schemes and compliance initiatives.
Lead clinics and provide medicines information/training to healthcare professionals.
Ensure adherence to local DSQS and other quality schemes.

Experience
Essential
- Strong interpersonal, communication, and IT skills required.
- Ability to work independently and as part of a team.
Qualifications
Essential
- Degree in Pharmacy and GPhC registration.
- Minimum 2 years post-graduate experience in pharmacy.
Desirable
- Member of the Royal Pharmaceutical Society.
- Independent prescriber qualification or working towards it.
Skills
Essential
- In-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
Desirable
- Experience within Primary Care Network setting.
