Job summary

An exciting opportunity has arisen to join GMMH Rehabilitation division as a trainee advanced practitioner.

The Advanced Clinical Practitioner will be a member of the multi-disciplinary mental health care team within Braeburn House which is a 28 bedded high dependency unit rehabilitation service . The role will also support the rehab senior leadership team with developments and education across the division.

Trainee Advanced Clinical Practitioners will develop into a autonomous independent practitioner who will lead in the care delivery in advanced sills in assessment, diagnosis, formulation and treatment. In addition to these advanced clinical leadership skills there will also be an expectation that the successful candidate will lead on research, education and training across the service.

Main duties of the job

Ability to practice at an advanced clinical level and exercise high levels of judgement, discretion and decision-making in clinical care.Provide clinical leadership, expert practice, advanced knowledge, and strategic direction on all aspects of rehab mental health .Providing clinical leadership and management leading to the provision of safe, quality, effective care.Improving workforce knowledge and practice through the provision of an inpatient education and trainingLeading improvements in the quality of care and safety through service development, evaluation and audit.

Job responsibilities

Job Summary/PurposeThe Advanced Clinical Practitioner will be a member of the Rehab Senior Leadership Team working across the whole division when required, based at Braeburn House for a minimum of 2 days per week to support MDT working with our partner organisation, Elysium health care The Advanced Practitioner will be responsible for assessment, formulation and prescribing of care planning including discharge planning within agreed protocols. The post holder will be recognised as an expert practitioner in an identified field of practice (including non-medical prescribing), and will advise the multi-disciplinary team on expert, safe and effective models of care and treatment.They will provide clinical leadership and management leading to the provision of safe, quality and effective care. They will also lead on Physical Healthcare agenda for the Division and will support the improvement of workforce knowledge through inpatient education and training programme.They will take a lead in educating and facilitating all other professions within Rehab services to meet the care needs of service users and their carers through the promotion of Recovery focused principles and philosophy.They will always act as an advocate for service users, carers and relatives and provide appropriate support and advice to carers including referral for a carers assessment.In collaboration with other senior staff, they will ensure that clinical practice is patient centred and research based in accordance with professional practice, guidelines and national nd local benchmarks.They will regularly undertake audits and service evaluation to inform service improvement using an evidence-based approach to draw on best practice. When necessary, they will support and facilitate colleagues in research and clinical audit to improve effectiveness and quality of patient care and participate in surveys as required.On the successful completion of the ACP course thepostholder will step into an 8a Advanced Clinical Practitioner role.
