Nurse Specialist/Team Leader
(Band 6)

Job summary

We are looking to recruit to an exciting Physical Healthcare Practitioner (PHCP) role.

To be the professional lead for nurses in the designated specialty, leading on professional development and ensuring the delivery of robust evidence based practice.

To participate in the establishment of systems assisting the Team Manager in the development of clinical practice, to ensure that the developments are embedded within the team and are sustainable.

To ensure high professional standards of nursing practice in accordance with the NMC code of conduct, with professional accountability to the Nursing and Governance Directorate

To lead the planning, implementation and participation in training and educational activities relevant to Practice Development for nurses in the designated specialty.

To support the ward or team manager and other senior clinical staff in establishing and maintaining high standards and continual improvement within the established governance framework.

To undertake ongoing audit of care and service delivery, underpinned by research and evidence-based practice. Undertakes research as part of the role.

To work as a non-medical prescriber / or provide a governed therapy within the NMC framework and Trust protocol, in accordance with qualification and competence to practice.

To provide nurse led diagnosis and treatment planning.

Main duties of the job

Through supervision and co-working in complex cases, supports nurses to formulate, implement and evaluate interventions with individuals that are person centred.

Takes the clinical history of patients and undertakes assessments specific to the area of professional practice and expertise, to provide nurse led diagnosis and treatment planning.

Demonstrates and provides in-depth knowledge and consultancy for nurses and to mainstream services which are providing support to patients and carers with specialist needs.

Provides direct clinical care interventions to support the development of practice in highly complex clinical cases.

Undertake risk assessment , and risk management for individual clients.

Undertakes the role of non-medical prescriber.

Analyses and interprets a range of complex facts and situations when assessing patient conditions in order to develop appropriate intervention plans.

Responds appropriately to crisis situations, where necessary seeking advice and guidancefrom other senior professionals. Implements prescribed strategies and interventions tomanage the crisis effectively.

Develops and co-ordinates training programmes for members of the clinical team, including students.

Leads the development of policies and procedures relating to own specialist area and ensures robust implementation, monitoring and evaluation.

Lead nurse in clinical care pathway development and implementation within own specialist area.

Person Specification

Knowledge

Essential

  • Demonstrable knowledge at post-graduate level of physical healthcare
  • Advanced knowledge and understanding of physical examination and assessment
  • Substantial knowledge of working at Band 6 level or above utilising clinical assessment skills
  • Knowledge of national policy and guidance in relation to aspects of physical health
  • Knowledge and experience of clinical audit and research
  • An understanding of clinical governance and the application of the clinical governance agenda in the NHS
  • Knowledge of incident reporting and the importance of incident investigation

Desirable

  • Previous knowledge of mental health care or working within a mental health or learning disability environment
  • Knowledge of mental health problems (e.g. specific mental illness, personality disorder, eating disorders etc.).
  • Evidence of further education in research or audit

Skills

Essential

  • Advanced skills in physical examination and assessment
  • Appropriate technical skills (e.g. venepuncture, ECG monitoring etc.).
  • High level of record keeping and documentation
  • High level of interpersonal and communication skills
  • Ability to plan, set objectives, priorities and review performance
  • Computer and keyboard skills and use of Microsoft packages

  • Participated in the delivery of teaching or education packages to patients, carers or staff

Experience

  • Extensive clinical knowledge and experience

Qualifications

Essential

  • First level Registered Nurse Adult (RNA) or Registered General Nurse (RGN) with current NMC registration and eligible to practice, or Registered Allied Health professional (AHP) with current HCPC registration and eligible to practice
  • BSc Nursing (or AHP equivalent), or post-registration specialist course at BSc level
  • Post-registration teaching and assessing qualification (e.g. Mentorship, ENB 998 or City and Guilds 730)
  • Post-registration Physical Assessment Skills (Adult) Level 6 qualification
  • Independent prescribing Level 6 qualification (or willing to work towards within 12 months)
  • Evidence of continual professional learning and development

Desirable

  • Registered Mental Health Nurse (RMN) or Registered Nurse Learning Disabilities (RNLD) with current NMC registration and eligible to practice
  • MSc Nursing (or AHP equivalent) or post-registration specialist course at MSc level
  • Post-registration Advanced Physical Assessment Skills and Disease Management Level 7 qualification (or equivalent)
  • Advanced Pathophysiology Level 7 qualification (or equivalent)
  • Clinical Assessment of Patients with Long-term Health Conditions Level 6 qualification (or equivalent)
