Are you a compassionate Registered Nurse RN seeking a new opportunity. We have Community Staff Nurse positions available within the team, based at Bridgwater.

Our service covers the urban and rural areas of Bridgwater Bay. These positions are dayshifts couple of twilight shift over a month and one in four weekends which include additional unsocial hours enhanced payments. There are no night shifts. We offer flexible working solutions where possible to ensure a work life balance and part time hours will be considered.

You'll need to be a car driver with your own vehicle to visit patients. We offer a generous mileage allowance to cover the use of your car for NHS business, which includes both mileage and depreciation.

Main duties of the job

Community Nurses work as part of a team providing high quality and compassionate care to individuals. They are autonomous practitioners who undertake a holistic assessment and write a personalised care plan following the assessment for individual patients in their own homes. They are responsible for safe delegation, evaluation of the care plan and discharging patients. They also have responsibility as a named nurse for individual patients. Teaching and supporting Assistant Practitioners, HCAs, students, carers and patients, liaising directly with all other Health and Social Care providers to ensure that patients needs are met

You will develop a broad range of nursing skills, including IV medication administration, palliative and end of life care, complex wound management, catheter and continence care, diabetes management, PICC line care, medication administration, and drain tube management.

You will need to make autonomous clinical decisions and create holistic care plans, with the support of your colleagues and Team Leaders. Comprehensive training and guidance will be provided by our two dedicated Clinical Lead nurses.

Job responsibilities

The band 5 staff nurses are autonomous practitioners who undertake a holistic assessment and write a personalised care plan following the assessment for individual patients in their own homes. They are responsible for safe delegation, evaluation of the care plan and discharging patients. They also have responsibility for the following.

Acting as a named nurse for individual patients Teaching and supervising Assistant Practitioners, HCAs, students, carers and patients Liaising directly with all other Health and Social Care providers to ensure that patients needs are met Manage caseload in absence of the band 6 Team Leader Have awareness of budgetary requirements to include the management of Equipment and resources within agreed budget parameters Organise their workload ensuring that visits are planned according to priority and location Take an active part in team meetings providing feedback on patients care and Contribute to the multi-disciplinary planning of individual patient care Documenting all care given in accordance with Trust policy Communicating effectively with all members of the district nursing team and wider multi-agency primary care team Having an understanding of the roles of different agents within the team to ensure efficient communication that optimises patient care Respecting the diversity of individual patients and colleagues. Qualifications

  • Registered General Nurse
  • Level one Registration with appropriate registration with NMC as per post requirements
