An exciting opportunity has arisen for
an 8-month fixed term or secondment for a B5 Mental Health Practitioner the
Worcestershire Neighbourhood Mental Health Service covering the Droitwich
locality.
37.5 hours during Monday to Friday.
Band 5
This role allows the possibility of
hybrid working, meaning you will be required to commute to the stated base 2 or
3 days per week but allowing you to based within any commutable distance for
the remainder of your week. We would need to discuss the suitability to fit
this around the service at interview.

Main duties of the job
Will work as a
key member and resource of the MDT core team.
This post will
offer a challenging and exciting opportunity to gain experience of working
across North Community Mental Health Service, developing relationships with
Primary Care, partner agencies and wider mental health services.
You will have
the opportunity to design and deliver evidence-based recovery focused mental
health interventions and groups. Knowledge
of delivery psychoeducation within a group environment and 1:1 would be an
advantage.
You will have
experience of working with this user group in community settings.
You will hold a
blended caseload of common mental health disorders, severe mental
illness patients, Primary and Secondary, including those with emotional needs.
The ability to
work flexibly as the need of the service demands is essential.
Attend weekly MDT meetings, participate
in professionals meetings, signpost to adjunct services depending on patient
need.
Support and guidance to the carers of
patients accessing the services

Job responsibilities
Act as a role model to
colleagues, always seeking to maintain the highest standards of

Provide specific Psychiatric
Nursing interventions to allocated patients/clients and their relatives who are
referred to the service.
Work in conjunction with all
primary care services and partnership agencies to foster close
multidisciplinary and multi-agency working.
Work across all clinically
appropriate patients, 17-years-old and over, to deliver evidence-based
interventions.
Under the direction of Service
Lead and as part of the Community Mental Health Service, provide prompt local
assessment, triage, risk assessment, risk management, signposting, onward
referral and evidence-based interventions for people presenting with mental
health problems.
Assist with the teams patient
clinics and medication monitoring/administration.
Represent the
team on specific projects and working groups as required.

- 1 years post registration experience on an acute psychiatric ward or Post registration experience working with Adults in Mental Health
