Job summary

As a Patient Safety and Investigation Lead, you will be working both autonomously and in collaboration with the corporate and divisional teams to lead system level patient safety incident investigations.

Work with divisional and corporate teams to identify areas for improvement from patient safety events in line with relevant Trust learning response policies and procedures.

Be involved in continuously reviewing and improving various elements of the Trust's Patient Safety Incident Response Framework policies and plan.

Deliver and support the Trust to provide compassionate engagement and involvement of those impacted by patient safety incidents.

Lead on the systems and processes required to provide oversight of key patient safety compliance metrics, including Duty of candour.

Be jointly responsible for designing and delivering training for a wide range of staff on various learning response methods and patient safety science.

Main duties of the job

  • Continue to develop own expertise of patient safety and support the Trust to evolve the capacity and capability of patient safety expertise within the Trust. This includes linking in with national patient safety expert networks.
  • Lead and continually seek to improve the operational effectiveness of the corporate patient safety team and that resources are optimised to their greatest effect.
  • Contribute to and participate in the design and delivery of the Trust response to the national patient safety strategy.
  • Provide expertise in patient safety science including system analysis, just culture, human factors, and the national patient safety strategy, including the patient safety incident response framework (PSIRF).
  • Provide expertise in patient incident investigation (including SEIPS), as well as other patient safety learning response methodologies. This includes direct observation of care as well as exploring with staff and patients on what occurred with aim of identifying any gaps and improvement areas in provision of care.
  • Contribute to and participate in the design and delivery of training for a wide range of staff on various learning response methods
  • Provide and receive highly complex, sensitive, and contentious information to and from staff, patients, families, carers, and members of the public, including where there may be significant barriers to understanding and acceptance.

Qualifications

Essential

  • Educated to degree level
  • Educate to Masters Level or, equivalent level of experience working at a senior level within patient safety.
  • Systems based patient safety investigation training.
  • Post graduate qualification in patient safety science subject.
  • Evidence of continuous professional development.

Desirable

  • Trained After Action Review facilitator and/or trainer

Experience

Essential

  • Acute NHS Trust experience
  • Experience in leading incident investigations and report writing.
  • Excellent organisational, planning and time management skills with an ability to prioritise workload.
  • Ability to adopt a systematic approach to problem solving.
  • Experience of compassionately involving patients and families in investigations.
  • Experience of applying a systems based approach to patient safety investigations (specifically application of the SEIPS framework)
  • Experience and knowledge of applying human factors approach to investigations
  • Ability to receive, analyse, interpret, and present complex information from multiple sources.
  • Ability to present complex or contentious information to large groups.
  • Ability to adapt delivery of information to enable understanding by a wide range of people, including healthcare professionals, executives, external agencies, patients, and families.
  • Experience of formal and informal teaching.

Desirable

  • Working knowledge of hospital IT systems i.e., Datix, Epic
  • Knowledge and experience of quality improvement.
  • Knowledge and experience of quality governance, ideally in a clinical setting.

Knowledge

Essential

  • Working knowledge of Microsoft Office including Word, excel, Teams and ability to format and present documents in professional and consistent manner.
  • Able to work independently as well as collaboratively within clinical areas, specialties, divisions and corporately across the Trust.
  • Ability to be proactive, innovative and generate new ideas to help develop their role.
  • Experience of engaging all stakeholders, including patients, families and staff involved in incidents, to produce quality, thorough investigations that lead to quality improvements within agreed deadlines.
  • Ability to collect, compile, organise, interpret, and present complex data.
  • Understanding of the NHS Patient Safety Strategy.
  • Budgetary management and business planning; including workforce planning
  • Understanding of key line management policies and procedures

Skills

Essential

  • A high level of emotional resilience in relation to subject matter.
  • Ability to analyse and review all aspects of a patient's care pathway and provide specialist advice regarding recommendations of modifications to practices where necessary.
  • Ability to constructively challenge and question clinical practice of all members of the multi -disciplinary team.
  • Excellent written and verbal communication skills and ability to communicate effectively with a wide range of stakeholders, both internally and external to the Trust.
  • Ability to produce and deliver teaching sessions.
  • Ability to be flexible, responsive, motivated, and supportive.
  • Able to provide guidance, training and offer support to other staff members to develop their skills related to investigations and learning responses.
