We are looking for an enthusiastic community practitioner to join our team based in the Bracknell Forest Family Hubs. You will be working within the Bracknell area as part of the 0-19 universal services team to deliver core and targeted services to children aged 0-5 and their families. You will collaborate with colleagues across the Family Hubs and school nursing in the implementation of the Healthy Child Programme underpinned by the Solihull Approach model of working.
The position can be flexible working around core hours of 9:00am -5.00pm, with no shifts or weekend working.
We will ensure you receive individualised and effective support from both your health visiting line manager and colleagues within your team.
Main duties of the job

Participate in developing and implementing outcome focused care plans and evaluate care given, in partnership with clients.

Provide support to families using the Solihull approach model in behaviour management, parenting skills and delivering programmes of care to vulnerable families.

Facilitate Health Visiting well baby drop in clinics, health promotion activities and groups

Work as part of the team providing the Chat Health service which is a text based service for parents to access support and advice across the 4 BHFT health visiting localities.

You will hold a caseload of targeted children under the direction of the health visitor to support and work more closely with

You will supervise nursery nurses offering support and guidance

You will need to attend regular supervision sessions, clinical, safeguarding and management created to ensure you feel supported and are able to share experiences to learn from peers and develop your own skills and knowledge.
