Job summary

The post holder will take the lead role for the operational management of patient flow and safety through the hospital. You will work closely with Emergency Department colleagues, Trust Matrons and Ward staff to ensure the safe implementation of patient pathways to ensure Trust targets are met. The post holder will be responsible for the reduction in Length of Stay objectives in order to ensure optimal patient flow though departments working alongside the Discharge Lead and Matrons.

The post holder will be experienced in operational management in a senior nursing role within a Healthcare / NHS setting and with dealing with complex situations, conflict management and will have previous experience working within patient flow. Problem solving and being able to respond and adapt to a changing high pressure environment are key to this role.

The post holder will work with internal and external stakeholders to define service need and facilitate new patient pathways as required working alongside the Head of Patient Flow and Divisional Director of Nursing. The role requires innovative thinking to move teams and services forward to meet key objectives.

The post holder will be expected to lead and support the hospital @ night team alongside patient flow out of hours.

Main duties of the job

  1. Deliver timely and coordinated transfer of patients ensuring following correct patient pathways and meeting the 4 hour standard targets; Improve the communication between the flow and ward teams to ensure seamless, timely patient transfers
  2. Lead the day-to-day/ night flow decisions in the hospital
  3. Reduction in Length of stay by reducing internal delays
  4. Reduction in the number of non-criteria to reside patients working closely alongside the Discharge Lead
  5. Focus and drive on same day increased Pathway 0 discharges
  6. A reduction in ambulance handover delays by ensuring safe patient flow through the emergency department
  7. Supporting and leading the hospital @ night team to maintain safety for the trust at night
  8. Maximise the use of SDEC and admission avoidance services to avoid unnecessary admissions
  9. Supporting initiatives such as Criteria Led discharge and Criteria to Reside processes to improve weekend flow
  10. Oversight of Escalation areas to improve flow and patient care and experience
  11. Increased flow through the discharge lounge
  12. Increase in discharges before 5pm to improve flow
