Job summary

An opportunity has arisen for an experienced Urgent Care Practitioner to work alongside our other Urgent Care Practitioners compromising of the Duty Doctor, Physicians Associate and Nurse Practitioner.

We are looking for an autonomous practitioner who can deliver a high standard of clinical care, prescribing is not essential.

Main duties of the job

Can you make professional autonomous decisions?

See patients with undifferentiated and undiagnosed problems and make an assessment of their healthcare needs?

Screen patients for disease risk factors and early signs of illness?

Make differential diagnosis using decision-making and problem-solving skills?

Develop care plans for health, with an emphasis on preventative measures?

Work collaboratively with other healthcare professionals and disciplines?

The successful candidate will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of patient care. The successful candidate would join our well-established medical practice and expanding clinical team in a friendly and supportive team environment.

The ideal candidate will have excellent patient skills, the ability to work at a face pace whilst maintaining attention to detail, and work well in a team.

The practice is based in the beautiful rural Gloucestershire countryside, we look after just over 6700 patients. We are part of the TWNS PCN, with Mythe Medical Practice, Church Street and West Cheltenham practices and Newent Surgery.

We are a well-established practice team including 2 GP Partners, 3 Salaried GP's, 1 Nurse Practitioner, 1 Physicians Associate, 2 Practice Nurses, 1 Healthcare Assistant, 1 Clinical Pharmacist, 1 Pharmacy Technician, 1 MSK Physiotherapist, 1 Social Prescriber, 1 General Practitioner Assistant, experienced Reception, Administration and Dispensary Team.

Job description

Job responsibilities

To practice as an Urgent Care Practitioner and demonstrate advanced clinical competence.

Provide a point of contact within the Practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan.

Assess, plan, treat and refer patients appropriately, carrying out consultations in the surgery or the community as an autonomous practitioner.

Using own clinical judgement to diagnose and treat patients either face to face consultation, clinical telephone triage and other mediums.

Making appropriate referrals to primary, secondary and social services care, following practice (and/or locally agreed) pathways and protocols.

To recognise the need for and initiate where appropriate relevant diagnostic investigations, using their skills in this area to take the correct course of action.

Understanding local safeguarding referral process and be aware of appropriate referral mechanisms for children and vulnerable adults

Ensure complete and accurate documentation of every patient contact, including advice given. Records must be accurate and reflect the clinical activity in accordance with NMC guidelines on clinical record keeping.

Minor illness assessment, diagnosis and treatment with access to seek advice, review and onward referral within clinical teams at Staunton and Corse Surgery.

To develop care plans for patients when required (as developed and supervised with the Partners)

Be involved with continuity of care of our patients, e.g. end of life care, chronic disease management.

Qualifications

Essential

  • Registered with a professional body as a paramedic, physicians associate or as a nurse
  • Advanced clinical practice skills
  • Management of patients with complex needs

Experience

  • Knowledge of public health issues both in wider and local practice area
