An exciting opportunity has arisen for a dynamic and enthusiastic registered nurse to join the award-winning Diabetes and Endocrinology Team in the role of Diabetes Specialist Nurse

The Diabetes Specialist Nurse will be part of a wider multidisciplinary team which has a strong commitment to improving standards of diabetes and endocrine care locally. We are looking for an individual with a passion for diabetes who is motivated, willing to develop their knowledge and drive-up standards of care locally.

A significant part of the role will involve working autonomously providing inpatient diabetes care and education at the trust hospital sites. Also supporting the diabetes emergency phone line and providing support to local health care professionals and outpatient services as needed.

Individuals will need to be flexible and able to work shift patterns/weekends. Training and support will be provided to successful applicants.

Main duties of the job

  • Participate in review of diabetes inpatients and or nurse led clinics using relevant protocols whilst exercising judgement, discretion and decision making in clinical care.
  • Discuss complex patients or those for specialist investigations with a senior member of the diabetes and endocrinology team as appropriate.
  • Liaise and support primary and secondary care clinicians with diabetes and endocrinology clinical care and management, including supporting the diabetes emergency phone line on a rota-based system which will be accessed by health care professionals and patients.
  • Be flexible and able to work appropriate shift patterns, including weekend work if needed. Communicate effectively with all members of the diabetes multi-disciplinary team.
  • Establish and maintain continuing relationships with patients, cares and relatives which are essential to providing effective long-term care.
  • Support patients and their families communicating sensitive condition related information and ensure they receive required information to enable them to participate in their care delivery
  • Support patients and their families during diagnosis of diabetes and treatment ensuring understanding of their diabetes and any related complications
  • Engage with patients and carers to gain views on service delivery and development
  • Communicate complex and sensitive information to other health care professionals in primary and secondary care.

Job responsibilities

CLINICAL & PROFESSIONAL RESPONSIBILITIES

  • Maintain Professional Registration
  • Adhere to relevant Code of Practice of Professional body
  • Autonomously assess triage and diagnosis diabetes and endocrinology conditions and review those with longstanding diabetes/endocrine conditions as inpatients or outpatients, recommending or initiating appropriate treatment within agreed protocols and guidelines.
  • Order, undertake, interpret and act upon diagnostic tests and results for patients with diabetes or endocrine conditions
  • Assess, plan, co-ordinate and implement and evaluate evidence-based diabetes care in conjunction with relevant health care professionals.
  • Undertake a number of specialist diabetes procedures which will include management of diabetic inpatients, delivering diabetes education, insulin initiation and titration, oral hypoglycaemic initiation and titration, managing patients with technology and diabetes.
  • Effectively manage own workload.
  • Involve patients, carers and relatives wherever possible in the planning, implementation and delivery of diabetes and endocrinology specialist care.
  • Undertake nurse prescribing if considered relevant.
  • Ensure compliance with specific regulations in regard to diabetes/endocrine drugs and administration and record keeping within area of work.

ADMINISTRATIVE RESPONSIBILITIES

  • Ensure compliance with specific diabetes protocols, standards and clinical paperwork including patient experience satisfaction and diabetes related surveys.
  • Undertake and provide telephone consultation/support as appropriate to patients and health care professionals

TEACHING & TRAINING RESPONSIBILITIES

  • Provide specialist knowledge, advice and support to colleagues, health care professionals, patients, carers and relatives around diabetes and related conditions.
  • Contribute to the multidisciplinary education and training provision/programme for health care professionals involved in caring for patients with diabetes across organisational boundaries.
  • Develop co-ordinate and participate in diabetes/endocrine education and training programmes and evaluate their effectiveness.
  • Raise awareness of diabetes and related conditions through health promotion activities and organised mentorship, training and evaluation to students and new starters within the trust events throughout the trust and wider community.
  • Provide support and education to patients, allowing them to develop self-management skills as appropriate to their diabetes.
  • Provide verbal and written advice to patients and carers with diabetes/endocrine conditions, around care management as required in hospital and in preparation for discharge.
  • Facilitate an appropriate learning environment for students and junior staff providing direct supervision.
  • Participate in regular clinical education sessions within the diabetes unit.
  • Identify, contribute to, or lead on some aspects in the development and dissemination of evidence-based guidelines, standards and protocols as required.
  • Develop and maintain required specialist clinical skills in the assessment and treatment of patients with diabetes and related conditions.
  • Participate and contribute to regular MDT and clinical supervision sessions as and when required Attend annual appraisal to discuss and identify training and development needs as part of a Personal Development Plan.
  • Support diabetes specialist nursing colleagues in their continuous professional development including peer and clinical supervision
  • Update and maintain own clinical knowledge and competence by attending post education courses, mandatory training, study days, workshops and conferences to keep up to date with best clinical practice.
  • Maintain regular links with other Diabetes Specialist Nurses at both local and national level.

LINE MANAGEMENT/SUPERVISORY RESPONSIBILITIES

  • With the Diabetes Nurse Consultant and the other members of the team, contribute to the development of agreed protocols, integrated care pathways and audit tools that monitor practice.

RESEARCH & AUDIT

  • Adhere to policies, procedures and protocols relevant to diabetes care, demonstrating understanding of evidence based care, including where to obtain evidence.
  • Conduct/participate in national and local audit related to diabetes/endocrine care, evaluating and implementing best practice as appropriate.
  • Will participate in research projects alongside Diabetes Nurse Consultant
  • Attend local, national meetings and conferences to network and share best practice.
  • Ensue diabetes specialist nursing practice is evidence based and reflects audit and research outcomes.
  • Maintain clear concise, legible and contemporaneous patient records at all times in line with Trust policy and the Standard for Recording Keeping.
  • Have an understanding of internal and external assessments such as CQC, COSHH, and NHSLA.
  • Participate in the implementation of initiatives to improve adult diabetes/endocrine care and support practice development to ensure a high-quality service.

Person Specification

Qualifications

Essential

  • Registered Nurse with current NMC Registration
  • Evidence of continued professional development
  • Teaching and Assessing Qualification/Training, or equivalent experience

Knowledge & Experience

Essential

  • Substantial post registration experience within an acute setting
  • Substantial Diabetes experience within a clinical setting
  • Knowledge of the skills required to undertake the role
  • Knowledge of current professional and clinical issues
  • Knowledge of safeguarding procedures
  • Knowledge and experience of collating and presenting evidence for internal and external assessments such as CQC and NHSLA.
  • Evidence of quality improvement in patient care
