Job summary

The Frimley Health Foundation Trust Breast Cancer team are looking for an experienced, dynamic nurse, who is passionate about the patient experience and the delivery of quality, patient-focused care, to join our team as a Band 7 Secondary Clinical Nurse Specialist, on a full time, permanent basis.

The post will be mainly based at Frimley Park Hospital and the post holder will also be required to support the FHFT service at St Luke's Cancer Centre, Guildford, one day per week.

The post involves acting as a keyworker and advocate to patients diagnosed with secondary breast cancer, providing support and information throughout their cancer pathway, as well as participation in service development.

Successful candidates will be experienced nurses, with previous clinical specialism in breast oncology, at Band 6 minimum. Ideally candidates would also have some SACT and/or palliative care knowledge and experience in managing patients with advancing disease. Advanced communications skills are essential.

Main duties of the job

You will work autonomously, managing the caseload of secondary breast patients, but within and supported by an established and collaborative team of Breast Clinical Nurse Specialists.

You will provide professional and clinical leadership and expertise in the care of patients with secondary breast cancer, acting as a specialist resource to provide advocacy, information and support to patients, carers and staff.

You will liaise effectively with all key members of the multidisciplinary team, so as to achieve a synchronised specialist service, within a multi-professional framework.

You will work across primary, secondary, tertiary and community care settings, in the planning and delivery of services that support people with a diagnosis of secondary breast cancer.

You will also liaise with colleagues external to the Trust, within the Surrey and Sussex Cancer Alliance and National Networks, in the interests of service improvement and development.

You will take responsibility for own evidence-based practice and implement and add to the body of knowledge which supports nursing practice within the Breast cancer care.

To provide informal and formal teaching, e.g., academic courses provided by the Trust.

Job description

Job responsibilities

  • To be a point of contact/key worker for patients diagnosed with a breast malignancy at pre-diagnosis, during treatment, in the follow up period and when metastatic disease is diagnosed whether face to face or via the telephone
  • Utilise advanced communication and counselling skills when discussing diagnosis, prognosis and treatment options for metastatic breast cancer with sensitivity and expertise
  • Work autonomously and demonstrate advanced practice by managing a caseload of patients and demonstrate advanced practice in the holistic assessment, physical assessment and non-medical prescribing and evaluation of care promoting the philosophy of person-centred care for patients
  • Implement and manage telemedicine clinics and outpatient nurse led clinics which will include Enhanced Supportive Care clinics
  • To be responsible for maintaining high standards of patient care and contribute to clinical audit
  • To provide psychological support and symptom control for patients diagnosed with both early and advanced breast cancer in a clinic, ward setting and telephone support
  • Use patient feedback to develop relevant support structure including support groups
  • Visit thein-patient ward areas including ED/AAU and review patients, where appropriate, providing guidance and information relevant to their care
  • Provide specialist, expanded roles of diagnostic and therapeutic procedures where necessary
  • Provide specialist advice to support ward staff when patients are admitted ensuring all medical and nursing needs are understood and carried out in accordance with Trust standards and policies
  • Act as a leader, advisor and role model to less experienced nurses, ensuring appropriate care is planned, implemented and evaluated
  • Work collaboratively with the Acute Oncology Team, the Cancer Navigators/Support Workers and the Specialist Palliative Care team to enhance patient care and management.

Person Specification

Qualifications

Essential

  • Degree in nursing and current NMC registration
  • Breast/oncology/palliative care qualification
  • Advanced communication/Sage & Thyme/ Psychology Level 2/other Counselling qualification
  • Teaching or mentorship qualification

Desirable

  • Masters level qualification or working towards
  • Advanced Physical Assessment qualification or working towards
  • NMP qualification or working towards

Skills/Competencies

Essential

  • Previous Breast CNS experience, minimum Band 6
  • Evidence of good organisational/time management skills
  • Patient education/support/counselling skills
  • Aware of national cancer strategy and guidelines
  • Proven ability to work autonomously/independently

Desirable

  • Involvement in research and audit
  • Report writing, audit and service development

Professional experience

Essential

  • Relevant recent experience in Breast/Oncology/SACT/Palliative Care nursing at Band 6 or above
  • Evidenced experience of multi-professional working
  • Evidenced experience of running nurse led clinics
  • Evidenced experience in advanced communication/counselling/patient education
  • Proven management/leadership experience

Desirable

  • Utilisation of research-based evidence and recent use of audit
  • Evidenced experience of service development and improvement
