Job summary

We are looking for an enthusiastic and motivated Physiotherapist or Occupational Therapist to work in our fast-paced and well-established Front Door Integrated Discharge Team based at Queens Medical Centre.

We provide specialist service within Nottingham University Hospitals Emergency Department and the Assessment Areas as well as working as an integral part of the Frailty Pathway.

This is your opportunity to join the Clinical In-Reach team, providing a comprehensive and patient-centered approach to admission avoidance and supported transfer of discharge. Surrounded by the best clinical and support staff, you will be creating forward-thinking and innovative approaches to managing people with multiple pathologies and complex health needs working with our Nottingham University Hospital Trust partners.

You will receive ongoing supervision from a clinical member of staff directed and guided by your needs and be expected to participate in staff development programmes and support student placements.

Upon successful appointment, you will complete an induction period and our holistic worker competencies to support your development.

You will work as part of an internal Rota system between 7.30am - 8pm Monday - Friday (8am - 6pm on weekends and bank holiday)

Main duties of the job

The post holder will be working autonomously and alongside Nurses, Physiotherapists, Occupational therapists, and Assistant Practitioners as a multi-disciplinary team member to undertake comprehensive assessment and intervention to clients within the pre-admission areas of Nottingham University Hospitals Queens Medical Centre Campus to facilitate the patient's discharge or appropriate pathway identification

The post holder will need sound knowledge and experience in working with complex, frail, and older people that are able to manage a caseload that consists of a wide range of cases for people presenting with a plethora of multi pathologies, as well as challenging medical and social circumstances.

  • You will provide community knowledge and skills whilst assessing and recommending appropriate discharge support.
  • You will need to be dynamic, flexible, and have a wide range of critical thinking skills.
  • You will need excellent assessment and intervention skills in order to plan, develop and evaluate goal-focused rehabilitation
  • You will need excellent communication and interpersonal skills. You should be able to prioritise your workload, manage your time effectively, and work flexibly within the team.
  • You will need excellent supervisory skills, up-to-date knowledge of staff appraisal, and performance management experience

Job Purpose

  • To work as an autonomous practitioner, you will undertake comprehensive assessments to identify the appropriate service for the clients referred.
  • To carry out profession specific assessment and evaluation as required.
  • To work within the MDT to plan and develop discharge plans for adults with a wide variety of complex physical and mental health needs
  • To be responsible for continuing the on-going development of high standards of clinical practice with evidence-based focus within the service.
  • To work, as required within the urgent care service, reacting to emergency health and social care referrals with the aim of preventing admission to hospital.

Dimensions

  • This post has no budgetary responsibility; however, the post holder is responsible for demonstrating cost consciousness.
  • You will be required to work primarily with the Queens Medical Centre but also across different bases and community settings which may include clients own home, residential or nursing homes or hospital.

Key Responsibilities

Clinical

  • To be professionally and legally responsible and accountable for all aspects of your work, including the management of clients in your care.
  • To have the ability to gain valid informed consent and to identify through assessment clients who lack capacity and work within a legal framework with clients who lack the capacity to consent to treatment.
  • To undertake the comprehensive assessment and evaluation of the patients Physical and Mental health needs to develop individualised discharge plans/rehabilitation/reablement goals.
  • To undertake specific professional assessments to ensure the best outcomes for clients
  • To use clinical reasoning and a wide range of skills and knowledge to devise specialised programmes of care.
  • To be responsible for coordinating the safe and timely discharge process from a variety of settings, including hospital and community beds.
  • To undertake comprehensive evidence-based risk assessments developing risk management plans.
  • To provide specialist advice, education, and mentorship to other members of the team and colleagues regarding the clinical management of clients.
  • To actively engage with families and carers in order to ensure the best outcome for the service user.
  • To demonstrate flexibility, prioritise effectively and time manage own clinical workload. Be able to delegate appropriately.
  • To lead in regular team meetings and review meetings regarding client caseload.
  • To keep accurate and timely service user records to ensure continuity of care.
  • To participate in the supervision framework as a supervisee and a supervisor, including the appraisal process.
