Job summary

The Integrated Falls & Bone Health Service is a proactive community service in Wandsworth, which aims to reduce the incidence of falls and fractures in the older adult population.

The team is based mainly at St John's Therapy Centre or Queen Mary's Hospital and can offer home or community location based, outpatient sessions and group classes to patients in the Wandsworth locality..

The team comprises of Physiotherapists, Occupational Therapists, Exercise Facilitators, Therapy Technical Instructors, and Admin Support.

Main duties of the job

  • To work as part of a multidisciplinary therapy team delivering a community based rehabilitation service and exercise provision for clients with a variety of conditions under the supervision of qualified therapists as one to one sessions and in group settings.
  • To assess and manage own caseload of patients referred with a variety of presentations without direct supervision of a qualified therapist.
  • Carry out clinical tasks (such as instruction / supervision and prescription of individual exercise programmes) delegated by qualified staff, requiring a wide range of technical ability, manual therapy skills, as well as an excellent level of interpersonal skills.
  • To work under own initiative and without supervision to provide comprehensive and effective administration of own workload and group classes.
  • Maintain safety in the clinical area or rehabilitation space through monitoring of equipment maintenance and the environment.
  • Measure, order and adjust equipment (walking aids and basic ADL equipment)
  • To achieve and maintain competencies for this role.
  • Be part of a multidisciplinary team committed to providing a high-quality clinical service.

Clinical

To carry out assessments of allocated clients, planning and delivering a range of rehabilitation programmes both as one to one sessions and as part of group classes.

To carry out rehabilitation of clients in their own homes, day centres, the local environment, including outdoors, and within healthcare locations without direction or supervision of a qualified therapist.

To report to an allocated qualified therapist supervisor about treatments undertaken, progress made, outcomes of intervention and to discuss when necessary, modification of rehabilitation programmes as part of a structured clinical supervision system.

To carry out planned therapy interventions as directed by a qualified occupational therapist or physiotherapist for a range of clients, including those with complex presentations.

To observe any progress, deterioration or other significant changes in a clients condition or performance and to report these to the relevant therapist involved in the clients care.

To participate in the delivery of client group education and exercise programmes.

To undertake the measurement and evaluation of your intervention through the use of goal orientated treatment plans, outcome measures and clinical supervision with a qualified therapist in order to progress treatment programmes and ensure clinical effectiveness.

To work in partnership with clients, carers and the multidisciplinary team to ensure a timely, coordinated and individualised approach to intervention which respects clients choices and values.

To empower clients to take responsibility for self management during treatment period and following discharge, using knowledge of local resources to facilitate this as appropriate.

To organise clinical caseload in consultation with colleagues taking into account management of waiting lists and necessary nonclinical duties.

To ensure the safe and competent fitting / installation, use, and maintenance of therapeutic, mobility, rehabilitative and assistive equipment.

To demonstrate physical ability to carry out therapeutic interventions including manual assessment and treatment techniques and therapeutic handling.

Communication

To take referrals, chase referral information and deal with enquiries over the telephone in a sensitive manner, taking into account the need for confidentiality at all times.

To use skills of persuasion, motivation and explanation to encourage clients and carers to undertake treatment programmes which may be painful, challenging and/or time consuming and therefore require negotiation.

To complete onward referrals as necessary.

To communicate effectively both verbally and non-verbally with patients, and their carers to maximise the rehabilitation potential of the clients on your caseload. This may include communicating with clients and carers who are deaf, visually impaired, have poor memory, cognitive problems or for whom English is not their first language.

To convey information about prognosis and treatment to clients and carers in a sensitive and timely manner and using language they understand.

To work within an integrated multidisciplinary service maintaining close liaison with the multidisciplinary Team Leader, qualified therapy staff and other support workers within the service to ensure the smooth running of the team and to deliver holistic care packages to clients.

To communicate effectively and confidently with other health and social care colleagues, voluntary agencies and carers to expedite continuing care of clients.

To maintain timely, accurate and concise records of treatment which meet the documentation standards of the service.

Person Specification

  • NVQ 3 in Health or Social Care or equivalent academic qualification and /or equivalent level of experience of working as a therapy assistant (Physio / OT / Rehab assistant.)
  • Experience of working in an NHS / caring environment
  • Previous experience in elderly care working with older people's rehabilitation within hospital/ community setting
