Job summary

An Excellent opportunity has arisen within Hambleton and Richmondshire Services. We are looking for skilled and outstanding senior clinicians to join our Urgent Community Response (UCR) and Hospital at Home (H@H) service within the Growing the Friarage and Community Collaborative.

Consideration will be given to candidates who do not possess the level 7 advanced physical assessment skills qualification, however must possess the level 6 equivalent and be willing to work towards within the first 2 years of appointment .

Non-Medical Prescribing is essential.

You will have significant post-registration experience, be a non-medical prescriber and have advanced clinical skills to help you effectively manage a caseload of patients with highly complex needs. You will utilise your advanced clinical skills, non-medical prescribing and case management approach which will have a direct and positive impact on the person's ability to cope with an acute presentation with the aim to reduce unnecessary hospital admissions.

Experience of working in a community setting, working within an emergency assessment unit or rehabilitation would be an advantage and we welcome informal visits to the areas.

Successful candidate must have access to a vehicle for work purposes to meet the requirements of the role.

Main duties of the job

In the role of the Community Clinical Nurse Practitioner you will be a key member of the unplanned Urgent Community Response hospital at home Service providing an in depth assessment of nursing care and treatment for patients in their own home to prevent unnecessary admissions and/or facilitate timely discharge from hospital.

Hours of operation is 24 hours per day over 7 days per week within the UCR service and 8am - 8pm in H@H.

The successful candidate(s) will join a team of UCR & H@H Clinical Practitioners with a wealth of clinical knowledge and experience who will support and encourage you in your new post.

Qualifications & Training

Essential

  • To be an independent non-medical prescriber with demonstrable experience
  • To have completed the advanced physical assessment and disease management skills course (level 7) OR have completed the physical assessment and disease management skills course (level 6) AND be willing to work towards the level 7 course within the first 2 years of appointment.
  • Teaching and mentoring qualification
  • To hold a Diploma or Degree awarded by professional body.
  • Registered as fit to practice by the Nursing and Midwifery Council (NMC) as a registered adult nurse.
  • Professional knowledge acquired through degree supplemented by post graduate diploma specialist training, experience, short courses plus further specialist training to master's (level 7) equivalent level

Knowledge & Skills

Essential

  • Participate in regular clinical supervision
  • Effective people management skills.
