Job summary

Purpose of post:

• Offer a telephone-based information gathering service which supports clinical decision making to establish mental health presentation, identifying risk and vulnerability for children, young people and families. • Provide appropriate and advice and information to enable professionals and families to access the right support at the right time

Main duties of the job

The duties and responsibilities of the post will include the following,

Working in the context of a small administrative and support team:

Respond to referrals in a timely and professional manner, following service policies and procedures.

Conduct concise information gathering by telephone to establish mental health presentation and impact on daily life.

Contact with professionals, children and young people and parents/carers, providing accessible information with a clear rational. Manage referrals and appointments, effectively utilising the patient management system.

Accurately record contacts, demonstrating an expertise in concise record keeping.

Identify risk, vulnerability, and safeguarding concerns effectively and action.

Follow information governance guidance, ensuring confidentiality of client information.

Provide informed advice and signposting, demonstrating an understanding of available services and explaining rational.

Support onwards referrals to appropriate agencies.

Hold referral discussions with clinicians, providing concise information about mental health presentations. • Attend internal and external meetings, providing accurate and up-to-date information.

Contribute to data collection and reporting frameworks.

Northpoint are commissioned to provide part of the Open Minds Partnership (Calderdale CAMHS - Child and Adolescent Mental Health Service). • The service includes a single point of access (known as the First Point of Contact - or FPoC), which is the single point of entry for all mental health referrals for children and young people, as well as referrals for Autism and ADHD assessment. • The FPOC team also offers advice, guidance, and consultation to the children’s workforce to help them to support young people with emotional health difficulties

We have a unique opportunity for an Information Gatherer to join Northpoint. This role will involve working at our Calderdale First Point of Contact (FPoC) service in Halifax. You will play a crucial role in our mission to provide vital support to children, young people, and families dealing with mental health challenges. What you will be doing as an information gatherer

As an Information Gatherer, your primary responsibility will be to offer a telephone-based information gathering service that supports clinical decision-making for mental health presentations. You will be a crucial link in identifying risks and vulnerabilities for children, young people, and their families. Additionally, you'll provide valuable advice and information to help professionals and families access the right support at the right time.

What we're looking for in our ideal information gatherer

  • A minimum of one years’ experience of working with children and young people with emotional/behavioural issues
  • Excellent interpersonal skills
  • Experience of gathering information over the telephone
  • Resilience to deal with calls from young people, parents/carers and professionals which may involve emotional or distressing content
  • Ability to accurately record contacts, demonstrating an expertise in concise record keeping
  • The skills to be able to identify risk, vulnerability, and safeguarding concerns effectively and action

This role would be suitable for either 2 postholders fulfilling a job share working 35 over the 5 days (potentially 3 days and 2 days) , or a full-time postholder working 35 hours over 5 days. Please specify in your application which hours you would prefer.

Person Specification

Experience and Qualification

Essential

  • A minimum of one years experience of working with children and young people with emotional and or behavioural issues
  • Experience in carrying out assessment of risk, analysis of strengths and protective factors
  • Experience of identifying safeguarding concerns and acting appropriately
  • Able to deal with calls from young people, parents/carers and professionals which may
  • involve emotional or distressing content

Desirable

  • A relevant qualification in the field of mental health or similar

Skills and Competencies

Essential

  • Ability to provide advice, guidance and consultation to parents and or carers and professionals on matters of children's mental health and emotional wellbeing.
  • Ability to write positive and professional reports and letters e.g. referral letters, discharge
  • summaries
  • Able to deal with calls from young people, parents/carers and professionals which may
  • involve emotional or distressing content
  • Able to build and maintain relationships with others both internal and external to the
  • organisation whilst maintaining appropriate professional boundaries
  • Ability to plan and organise workload in line with targets and pressures
  • Able to work according to organisational policies and procedures
  • IT skills i.e. able to use Microsoft Office packages including Word and Outlook and video
  • conferencing such as Microsoft Teams
  • Ability to use IT based case management systems and databases
  • Excellent interpersonal skills, including communicating clearly and concisely at all levels
  • (both orally and in writing)

Desirable

  • Fluent in language other than English
