Are you an enthusiastic and dynamic Dietitian with excellent team-working skills? An opportunity has arisen to join our forward thinking and dynamic Dietetic community team across Hywel Dda to address malnutrition as part of our Malnutrition business case. The service has received significant investment in addressing malnutrition and as a result, this post offers an exciting opportunity to work collaboratively with the wider community team to address malnutrition in the community setting as well as supporting exciting and innovative project work such as self-screening for malnutrition, virtual clinics, digital platforms and new ways of working.

Main duties of the job

The post-holder will carry a complex caseload of community nutritional support patients and will also engage with multi-professional GP led team meetings to embed nutritional care and provide expert nutritional advice and support to the wider team on nutritional screening, referral pathwaysand nutritional care planning. Training and education will be a key part of this role. This post offers the ability for agile home working balanced with the opportunity to be based on site (at least one day per week). The post holder will undertake community visits, attend MDT meetings and attendanywhere or face to face clinics.

Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties. Qualifications & Knowledge

  • Recognised Degree in Dietetics with state registration
  • HCPC registered
  • Evidence of attendance on accredited specialist courses and or advanced training in relation to dietetics at postgraduate diploma level
  • Demonstrates consolidated clinical dietetic knowledge and skills in the field of adult dietetics
  • Has developed specialist knowledge as evidenced by dietetics qualification and additional formal learning / demonstrable experience in adult acute and community dietetics
  • Skills in behavioural approaches including motivational interviewing
  • Demonstrates good knowledge of, and ability to apply, evidence base in relation to dietetic practice and development of practice.
  • Critical appraisal skills : knowledge of current and emerging evidence base, national and professional policy, guidance and best practice in the delivery and development of the specialist area of dietitic work
  • Well established clinical problem solving skills
  • Demonstrates effective organisational skills including prioritisation of workload, effective caseload management
  • Well established teaching and presentation skills

Desirable

  • Clinical supervisory skills training

Experience

Essential

  • Demonstrate clinical experience in adult dietetics
  • Experience of working in acute, community and outpatient settings to deliver dietetic care
  • Experience of delivering training and educational sessions to staff and group education to patients.
  • Evidence of working as part of multidisciplinary teams.
  • Evidence of involvement in development work in dietetic services e.g. audit, implementing a change in practice, delivering a new dietetic intervention
  • Experience of clinical audit

  • Experience of delegating work to and supporting/ supervising support workers/ students/ junior dietetic staff
