Job summary

An exciting opportunity has arisen for a Band 6 Registered Mental Health Nurse to join the Salford Home Based Treatment Team's Hollybank crisis bed and Listening Lounge service.

The Listening Lounge in Salford is a 24-hour friendly and supportive community space open to anyone in Salford who has been referred by a health care professional and requires an urgent assessment of their mental health needs without the need for medical treatment.

Our aim is to offer a 1:1 holistic assessment of an individual's mental health needs from qualified mental health practitioners and support staff in a relaxed and comfortable environment, avoiding long waits in emergency departments.

The Listening Lounge provides brief psychosocial interventions in a calm environment to help identify and promote positive coping strategies, and complete referrals to identified and appropriate services, and signpost to other local support services.

The postholder will also be involved with assessing, planning, implementing, documenting and evaluating individual programmes of care and risk assessment for designated clients who utilise the two crisis beds at the Hollybank service, who will remain under the care of the Home Based Treatment Team .

The team operates a 24/7 service, therefore there will be a requirement to cover shift work. Flexible working requests can and will be supported however possible.

- To effectively screen and gatekeep referrals to urgent care services, ensuring that the least restrictive options for treatment are offered

- To provide comprehensive assessment of a service users mental state and social needs which would aid the completion of a collaborative care plan for community management.

- To provide comprehensive risk assessment, formulation and management as part of the service user's journey.

- To act as keyworker for service users utilising the Hollybank crisis beds , with support from the multi-disciplinary team.

- Cover the duty / shift coordination role at times.

- Liaison with other agencies involved in a service users care.

- Lead on the discharge or transfer of care for our service users to appropriate agencies.

- To support and supervise junior members of staff.
