Job summary

We are looking for a dedicated and pro-active nurse or registered practitioner to work as part of our well-established Street Triage Team. The team will work closely with the Cleveland Constabulary but will be aligned to the wider crisis services already provided by Tees, Esk and Wear Valleys NHS Foundation Trust within the locality.

The team will work in partnership with Cleveland Police to identify people with mental health needs and support them into appropriate services, creating effective partnerships and tackling stigma.

Main duties of the job

The team will offer advice to the police to assist their decision making and ensure people have access to the correct support, ensuring they get the right help at the right time and the right care from the right person.

If it is identified that the person is in a mental health crisis, then the STT practitioner will be contacted by the police, so that the STT can conduct a full triage with the person, to determine the most relevant response, based on need and risk.

If the STT clinician cannot triage via the telephone, they will need to conduct a face to face triage or assessment and then devise a plan following this assessment which will consider the needs and safety of the person in crisis.

Person Specification

Qualifications

Essential

  • Current professional registration with a recognised professional body
  • Evidence of Continuing Professional Development
  • MIP, FLIP, ENB 998 Teaching & Assessing in Clinical Practice or City & Guilds 730 Teaching in Adult Education.
  • Recognised sign-off mentor (or equivalent) or must be achieved within agreed timescale.
  • Key skills in literacy, numeracy and ITQ level 2 (or equivalent)

Desirable

  • Leadership or management qualification
  • Post-graduate qualification in a relevant area

Experience

Essential

  • Significant experience working with people with complex mental ill health, behavioural and social issues, in a community setting
  • Experience in assessing and treating mental health patients in crisis.
  • Working collaboratively with service users and their families/carers.

Desirable

  • Leadership or management experience

Knowledge

Essential

  • Understanding of relevant legislation (e.g. Mental Health Act, Mental Capacity Act).
  • Detailed understanding of Safeguarding and its application in practice.
  • Clinical Risk Assessment and Management and its application in practice.
