This part-time sessional vacancy presents an incredible opportunity for GPs with an interest and passion in caring for older and frail patients who also possess the drive to help further develop a high-profile service care within the community, to join our innovative, forward thinking, national award-winning team.
The Jean Bishop Integrated Care Centre, Frailty Support team (ICC-FST) has been operational since May 2018, and in that time provided innovative, anticipatory patient tailored care to frail residents of Hull and the surrounding areas, including those residing in care homes. The centre has already been recognized for national awards and continues to receive outstanding patient feedback. The ICC-FST is the leading case study in the Ageing Well NHS Long Term Plan. The team working at the ICC-FST feel privileged to be a part of delivering transformed care for older people in Hull.
We are looking to expand our team of General Practitioners with Extended Roles (GPwER) in Frailty who support the work of our multidisciplinary team, in looking after the most frail and complex patients residing within either Hull or East Riding. Our multidisciplinary team is inclusive of: GPwER in frailty, Consultant Geriatricians, Advanced Nurse Frailty Practitioners, Clinical Pharmacists together with Occupational and Physiotherapists, Mental Health nurses, Social Workers and Carers support team.
Main duties of the job
This is an amazing and varied opportunity for General Practitioners passionate about caring for older and frail patients to add to their portfolio of experience and also provides the flexibility to support any other existing working commitments. We are able to offer a minimum of 2 sessions which can be carried out in one working day.
In this role you will work across all work-streams supported by the ICC-FST; assessing and pro-actively planning the care and treatment of older people and those with frailty, working closely with the multi-disciplinary team and community services to ensure patients receive holistic and comprehensive care and support and also our soon to be commenced, Virtual Ward Service.
The abilities to communicate professionally and effectively whilst in a caring manner with patients and their carers together with experience of working within a multi-disciplinary team are also essential, working experience/knowledge of local pathways and referral procedures would be advantageous.

Qualifications
Essential
- Full and current unrestricted GMC (UK) registration on the GP Register
- A medical degree qualification
Desirable
- A teaching qualification e.g post graduate diploma in post graduate education
- A post graduate qualification in older peoples care, e.g. Diploma Geriatric Medicine (or willing to work towards)
Experience
Essential
- Post GMC registration training in care of older people
- 3 Years or more post GP specialist registration experience
- Experience of supervising clinical staff to ensure clinical standards are met.
- Experience of continuing professional development.
- Evidence of attendance at courses or self directed learning to meet learning gaps identified through the professional development plan and through annual appraisal
- Experience of conducting clinical audit
Desirable
- Experience of designing, organising, teaching and training healthcare professionals and teaching patients
Knowledge & Understanding
Essential
- Up to date knowledge of all the latest guidelines and evidence in older peoples medicine.
- Understand the importance of evidence based practice and clinical effectiveness
- Understanding of clinical governance Sound clinical knowledge
- Ability to use the evidence base and clinical audit to support decision-making
Desirable
- Experience of SystmOne, Emis and Lorenzo clinical systems
