Do you have what it takes to give the population of Heywood, Middleton and Rochdale the best possible chance of remaining in their own residence whilst also allowing them to receive the best possible care?

We are seeking to recruit an experienced and motivated individual to the role of Advanced Practitioner to work within the HMR Rapid Response Team, providing 2hr urgent community response to the population of Heywood , Middleton and Rochdale.

We are a community based, multi-disciplinary team of skilled professionals, who provide holistic care and support to enable people to remain in their own home and avoid unnecessary admission to hospital.

This is an exciting opportunity to join a multidisciplinary team encompassing both health and social care alongside links with community, primary and secondary care. We are seeking an individual with excellent clinical skills, a good working knowledge of acute conditions as well as Frailty and Elderly Care expertise. The successful individual is required to have strong communication and leadership skills and will be a dynamic and forward-thinking Clinician with a progressive approach to enhancing patient care and safety.

Main duties of the job

An autonomous practitioner, the post holder will take an active role in the advanced assessment, management and treatment of patients over 18 years of age. The practitioner will provide comprehensive healthcare sensitive to the needs of the population, including advanced clinical assessments, take history, diagnose, plan treatment, prescribe and evaluate. Leading the multi - disciplinary team across the primary, secondary and social care interfaces within HMR you will be responsible for coordinating and meeting the health care needs of people at risk of hospitalisation or requiring a more intensive level of care in their home environment.

The post holder will provide specialised input as an independent and interdependent practitioner developing and leading relationships with members of the multi-professional team allowing for consultation and collaboration in a variety of health care settings. This will be to prevent unnecessary acute hospital admissions and keep patients in their own homes for longer and prevent premature admissions into long term care facilities.
