Job summary

You will join an experienced and dedicated team of mental health professionals, providing an efficient and effective service triaging referrals and undertaking assessments with people experiencing mental health difficulties.

Referrals are received from GPs and other external agencies; you will work as part of a multi-disciplinary team (including consultant psychiatrists), offering timely assessments and referring /signposting all patients on to appropriate services.

You will deliver care and support in accordance with all Pennine Care NHS Foundation Trust policies and procedures.

Main duties of the job

To triage all referrals received into the service.

To provide comprehensive biopsychosocial assessments and robust risk assessments for people referred to the service and to feedback to the referrer.

To make onward referrals to the most appropriate service to meet the needs of the individual following triage / assessment.

To identify the need for protection in line with the Trust's safeguarding policies and procedures.

To liaise and negotiate with other professionals, statutory and independent agencies to ensure the best possible service is provided to service users and their carers.

To involve family members/carers in the assessment process, with the service user's consent.

To undertake evidence based practice and to maintain accurate and contemporaneous clinical records.

Job responsibilities

The NHS long term plan and the Community Mental Health Framework (2019) outline an ambitious vision for the transformation of community mental health services across England. You will be joining Pennine Care at an exciting time as we respond to this challenge by implementing the Living Well model of care and support for people with complex mental health needs who are not accessing specialist services. We are re-designing existing services with new investment in clinical and non-clinical roles to create multi-disciplinary teams that offer person-centered, trauma informed and holistic support.

We envisage our existing Access function will change and integrate into the Living Well multi-disciplinary team. We are currently consulting with staff as we want to reflect a strengthened emphasis in current roles on multi-disciplinary working, particularly with the Voluntary Sector, Housing, Employment and Substance Use partners.

We wish to emphasise that we are not recruiting to at-risk roles; all advertised positions are secure and permanent. Instead, you will join us at a point where we are working together with our staff and people with lived experience to develop new ways of working, building on existing roles, and with opportunities for you to make an active contribution to the development of our future offer to the communities of Oldham.

Person Specification

Education/qualifications

Essential

  • RMN/RNMH/Social Worker equivalent
  • Completion of Preparation for Mentorship / Practice Teacher / Fieldwork Educator qualification or willingness to complete
  • Evidence of on-going CPD

Desirable

  • Degree - or working towards completion.
  • Non-medical prescribing qualification
  • Formal teaching qualification.
  • Further training in brief psychosocial interventions.

Experience

Essential

  • Significant post registration experience.
  • Proven experience of working with clients with complex needs.
  • Experience of working with individuals who present to mental health services in crisis and have symptoms of mental health illness
  • Experience of working in multi-disciplinary teams
  • Experience of providing supervision to junior staff.

Desirable

  • Primary care mental health work experience
  • Experienced in the use of outcome measures such as CORE, PHQ9 and GAD7
  • Experience of working with a psychological model of care
  • Experience of triaging referrals

Knowledge

Essential

  • Understanding of current mental health legislation, Safe Guarding and Child Protection Legislation
  • Awareness of NICE Guidance in the treatment of mental health problems and knowledge of treatments and interventions appropriate for individuals with mental health needs
  • Understanding of confidentiality and data protection issues.
  • Knowledge of psychosocial assessment and risk assessment/management processes

Skills and Abilities

Essential

  • Comprehensive mental health and risk assessment skills.
  • Ability to construct effective packages of care and risk management plans
  • Ability to deliver a range of evidence based interventions.
  • Excellent communication skills, both written and verbal
  • Comprehensive I.T Skills
  • Ability to form productive working relationships with service users, carers and multi-disciplinary staff

  • Skills in planning, delivery and evaluation of training
