An exciting opportunity has arisen for a Diabetes Specialist Nurse to join our friendly, forward thinking and supportive Community Diabetes Service that provides specialist clinical diabetes care to adults across Bury, Heywood, Middleton and Rochdale

The service provides support between the primary & secondary care interface, in preventing unnecessary readmissions. The service also provides support, advice, education and information relating to the management of patients, carers, the public, other health professionals & the voluntary sector.

The team is based at Moorgate Primary care centre Bury and Phoenix centre Heywood and offers specialist diabetes interventions to patients, families and carers across a range of community care settings.

Main duties of the job

The candidate must have current NMC registration first level nurse and current experience and knowledge of all aspects of the diabetes specialty. An independent prescriber is preferred or working towards or interested in gaining this qualification to initiate and adjust injectable and oral diabetes medication.

The successful candidate will be supported through a comprehensive induction programme, with access to clinical supervision and continuing professional development. Their contribution to future service development will be positively encouraged.

This vacancy is open to those who have a current NMC PIN to practice as an Registered Nurse or Midwife in the UK, or those who are currently enrolled on a course which means they expect to receive their full NMC PIN within the next 3 months. Please note an NMC PRN PIN is not a full NMC PIN and would not fulfil the essential criteria for this role.

Person Specification

Essential Qualifications/Experience

Essential

  • Registered Nurse with current NMC registration
  • Evidence of professional development at Certificate/ Diploma/ Degree level in adult Community Nursing e.g. District Nurse, Practice Nurse or Community Nurse
  • Mentorship/teaching or coaching qualification

Desirable

  • ENB 928/Warwick Diabetes Course or an equivalent
  • National Independent Prescribing qualification V300 or willingness to undertake training

Knowledge, Skills and Experience

Essential

  • Evidence of Continuing Professional Development
  • Assessment and analysis of complex needs of patient with diabetes.
  • Ability to advise on appropriate interventions, monitor, evaluate and adapt diabetes treatment as required.
  • Ability to develop educational resources that are evidence based to meet the needs of patients and carers
  • Willingness to acquire new skills & knowledge & undertake further training in line with the demands of the service provision
  • An ability to demonstrate a range of communication styles to various stakeholders
  • Good time management & team working skills
  • Good oral & written communication skills to related at all levels with patients, colleagues and members of the MDT
  • Ability to work independently
  • Ability to practice using behaviour change methods
  • Ability to deliver group education using a range of teaching techniques
  • Able to reflect and appraise own performance

Desirable

  • Understanding and knowledge of recent community developments
  • Ability to deliver diabetes structured education programme
