Wellington Medical Practice are looking for an Advanced Clinical Practitioner with a Paramedic or Nurse background to join our busy and dynamic team.

We will ask you to demonstrate high levels of judgment, discretion, and decision-making in the delivery of clinical care.

Joining our existing team you will provide advanced clinical skills in relation to assessment, diagnosis, treatment, and management of patients within the GP surgery and provide specialist clinical advice where appropriate.

Provide day-to-day acute care and long-term condition management, participating fully in the delivery of healthcare provisions in the primary care setting.

Main duties of the job

Autonomous practitioner who is able to work independently and in conjunction with other members of the MDT.

The ACP will understand and demonstrate evidence-based practice and have a broad knowledge of clinical governance, enabling them to work within their scope of practice.

The ACP will be an experienced caring practitioner and independent prescriber with highly developed clinical skills and be able to demonstrate critical thinking whilst working within professional boundaries

The ACP will work collaboratively with the MDT to support the delivery of policies and procedures

Participate in local initiatives to enhance service delivery and improve patient care

Take an active role in clinical supervision and reflective practice

Wellington is a progressive and innovative practice with over 15,000 patients in Telford and Wrekin CCG, rated good by CQC.

At WMP we recognise the changing and evolving health care climate and the value that Allied Health Professionals can add to patients care, improving the patient journey and ultimately outcomes. Currently we are keen to further recruit to our Multi-disciplinary team to widen our visiting service and onsite Advanced Urgent Care Clinics.

We currently have 4 GP partners working with a team of Advanced Practitioners, Nurse Practitioners/ Practice Nurses, Nursing Associates, First Contact Physios, Pharmacists and the enthusiastic reception/admin team to deliver innovative and quality patient care.

Job responsibilities

Be educated to masters level, this education will include the following core areas: comprehensive physical assessment of all body systems across the lifespan, history-taking and clinical decision-making skills health and disease, including physical, sociological, psychological, and cultural aspects of history-taking, public health, epidemiology, health education, and promotion

The ACP will also be an independent prescriber.

Contribute positively to innovative change, be flexible, responsive to new ways of working

Person Specification

Qualifications

Essential

  • Practice within the four pillars;
  • Advanced Clinical Practice
  • Leadership
  • Facilitation of Education and Learning
  • Evidence research and development
  • Registered with a professional body
  • Be educated to masters level
  • Be an Independent Prescriber
  • Paediatric post graduate training or equivalent
  • Considerable Primary Care experience

Desirable

  • Teaching post-graduate training or equivalent
  • Vaccine, trained.

Experience

Essential

  • 5 years post graduate experience
  • Knowledge of working in a primary care environment
  • Autonomous working
  • Reflective practitioner, experienced in clinical supervision management
  • Evidence of CPD, to maintain up-to-date and evolving competencies
  • Competent in secondary care/2ww/community-based referrals

Desirable

  • To be aware of audits and their place in primary care
