An exciting opportunity has arisen for a Band 3, Health Care Support Worker to join the Basingstoke Older Persons Mental Health Community Team
The team is a highly supportive and innovative team that aspires to provide a responsive, safe, effective, caring service that is well led.
Older Peoples Mental Health Services is looking for a motivated health care support worker to provide and deliver skilled interventions to older people with mental health needs within the Basingstoke district
The role involves providing post diagnostic support to those diagnosed with dementia supporting individuals with other mental health diagnosis to feel included in the community to continue to manage their own wellbeing and independence at home and provide support and signposting to families and their carers
To be a successful member of our team you will need to be motivated passionate and have a genuine interest in working with older people and be able to demonstrate a real commitment to providing high quality and compassionate care to patients and their families
Main duties of the job
To provide physical and psychological care to service users in accordance with the specified plan of clinical care and in line with the recovery model to service users who live in any part of the catchment area.
To carry out duties as designated by the team leader.
To provide support to the person responsible for the clinical care of the service user.
To accurately record service user information.
To provide support to relatives/carers.
